In short. The neurological and inflammatory signs that must not be treated as an aesthetic concern, each with the action it calls for. A list to use rather than to read.

Key points

  • One pattern is worth knowing because it is so easy to miss: a lid that is worse in the evening than in the morning, or that fluctuates with fatigue, can point to a condition affecting the connection between nerve and muscle rather than to an ageing change.
  • It is not an emergency, and it does need a medical assessment rather than an operation.

Most of this site is about a lid that has changed slowly and needs a decision rather than urgency. This page is the exception: it is the list of findings that change the priority. Each one is followed by the action it calls for.

Sudden onset, or one side only

A lid that comes down over hours or days, or a lid that is low on one side while the other is normal, is assessed urgently. The action is a medical assessment that includes the pupil, the eye movements and a neurological check rather than a cosmetic consultation. This is the most important item on the page.

Vision changes

Double vision, blurring that is new, a patch of lost sight, or pain when the eye moves are all reasons for urgent assessment. So is tenderness over the temple or scalp, particularly in an older patient, and jaw pain that appears when chewing.

Weakness beyond the eye

A droopy lid that comes with weakness in an arm or a leg, difficulty swallowing or speaking, a new severe headache, or a change in sensation on one side of the face is a neurological problem until it is proven otherwise. The action is emergency care rather than a clinic appointment, and the reason is that some of these conditions are treatable only within a narrow window.

A droopy lid that varies through the day

One pattern is worth knowing because it is so easy to miss: a lid that is worse in the evening than in the morning, or that fluctuates with fatigue, can point to a condition affecting the connection between nerve and muscle rather than to an ageing change. It is not an emergency, and it does need a medical assessment rather than an operation.

Inflammation and infection

A red, hot, painful lid with swelling, or a lid that is swollen with a fever, is an infection until proven otherwise, and it is assessed the same day. A lid that has become red and crusted at the margin is a different problem again, affecting the lid surface rather than its position, and it needs its own treatment before anything else is planned.

A low lid in a child

A lid that covers the pupil in a baby or young child is assessed promptly, because vision is still developing and the change can become permanent if it is not addressed in time. The window is limited and it does close.

Seek emergency care now if there is

  • A lid that dropped suddenly, or on one side only, particularly with eye pain.
  • Double vision, a loss of part of the vision, or a pupil that looks different.
  • Weakness in a limb, difficulty speaking or swallowing, or a sudden severe headache.

Why these findings sit outside the aesthetic pathway is set out in When a Droopy Eyelid Is a Medical Problem. The examination that looks for them is described in How a Droopy Eyelid Is Assessed.

Frequently asked questions

What is the single most important sign to act on?

A lid that has dropped suddenly, particularly on one side. It is the finding that most often separates an ageing change from something that needs treatment quickly, and it is the one people most often explain away. If it appeared in hours or days, treat it as urgent rather than as cosmetic.

Should I go to an emergency department or make an appointment?

If vision is affected, if there is double vision, severe pain, weakness elsewhere in the body or difficulty speaking or swallowing, emergency care is the right route. If the droop is new but there is none of that, an urgent appointment with a clinician who can examine the eye and its movements is appropriate rather than waiting for a routine cosmetic slot.

Can a droopy lid be caused by medication?

It can. Some medicines can produce a droopy lid, and a droopy lid can be an early sign of a general condition rather than a local one. That is one of the reasons a medication history is taken at the assessment, and one of the reasons a new droopy lid is not treated as a purely local problem.

Risks & Limitations

  • A lid that has dropped suddenly, or on one side only, is a medical matter rather than an aesthetic one and needs urgent assessment. Double vision, a changed pupil, eye pain, weakness elsewhere or a severe new headache require emergency care.
  • Eyelid surgery changes how completely the lid closes, so it can affect the surface of the eye. An eye that is already dry is a reason for caution and sometimes a reason not to operate.
  • Perfect symmetry is not something an operation can promise. Most faces are asymmetric before surgery and healing differs between the two sides.
  • Scars are permanent. They mature and fade over months but do not disappear, and the tissues continue to age after any operation.

When to Seek Care

  • A lid that has dropped over hours or days, or on one side only — seek urgent medical assessment.
  • Double vision, a loss of part of the vision, a pupil that looks different, or severe eye pain — emergency care.
  • Weakness in a limb, difficulty speaking or swallowing, or a sudden severe headache — emergency care.
  • After surgery: a lid that will not close, spreading redness, fever, or bleeding that does not stop.
